Poway Unified School District is required to make reasonable accommodations for any student with a disability that restricts his/her diet, when supported by a Medical Statement to Request Special Meals and/or Accommodations from a state licensed healthcare professional.
The California Department of Education only permits the following state licensed healthcare professionals to complete and sign the form: licensed physicians, physician assistants, and nurse practitioners.
The district will accommodate the special dietary needs of students who have been identified as having a disability as noted above.
There is a very wide range of conditions considered disabling so that is to be determined by the licensed physician, PA or APN with RXN.
The form must be completed in its entirety and returned to the student’s school administration office.
The form should be updated by the licensed healthcare professional, as appropriate, to reflect current medical/nutritional needs of the student.
USDA requires targeting Disticts with a free and reduced rate over 50%, not previously awarded Schools and the equipment must be purchases after the award notification.
To request soy milk for a non-disabling medical or dietary need, please complete and sign the Parental Request for a Fluid Milk Substitution form, and return it to your child’s school administration office.A Sponsor may, at its discretion, make substitutions for individual participants who are not "disabled persons" but are unable to consume a food item because of medical or other special dietary needs.Below are forms, resources and guidance to be used for special dietary needs.Your child may qualify for federally subsidized specialized formula given during school hours.The USDA Guidance Manual, "Accommodating Children with Special Dietary Needs in the School Nutrition Programs" explains the school foodservice role in providing meals to students with special dietary needs.
